Type 2 diabetes is no longer a disease of just adults, and it has become imperative that pediatric providers become well-versed in the management of pediatric type 2 diabetes and related co-morbidities.

ISPAD T2D SIG – Project Planning Meeting #1 Minutes

Title: Mapping the global landscape of Y-T2D diagnosis and management
Proposal: Narrative review and survey of the diagnosis and management of youth-onset type 2 diabetes (Y-T2D) across different geographic locations, including high and low resourced communities. Areas to address include the tools and strategies being used to delivery care and health equity and challenges faced within different communities. Potential questions include, but are not limited to, provider access to: diagnostic tools, device-assisted technologies (e.g. continuous glucose monitors and insulin pumps), pharmacological management, the health care system infrastructure (adult vs pediatric focused/ multi-disciplinary clinics).
3. Discussed project content, scientific rationale, scope, technical feasibility, organization, and timeline.
a. Scientific rationale – Limited data on practices across regions. Management data would help to increase awareness and advocacy and help to reduce healthcare stigma associated with Y-T2D. Younger individuals under-represented in health practice/ implementation research.
b. Unmet Need – Research gap - May also be interesting to examine how resources differ by region and the health economics of treatment Y-T2D
c. Scope – include questions on
i. Age range managed in practice - up to 21 vs 25 y vs 18 y
ii. How are centers funded
iii. diagnostic tools
iv. device-assisted technologies (e.g. continuous glucose monitors and insulin pumps)
v. pharmacological management
vi. the health care system infrastructure (adult vs pediatric focused/ multi-disciplinary clinics
d. Technical feasibility and Limitations of Survey
i. Denominator unknown.
ii. UK and other surveys defined the patient experiences especially during transition. This survey would aim at providers and management practices.
iii. Surveys have low response rate - <30% - how can we improve and actively promote study. Can advertise and network to colleagues. Survey multiple stakeholders (not just limited to ISPAD) to gain wide range of responses from all providers that care for Y-T2D. Network and share survey with multiple colleagues and societies. Reach out to leadership of societies and authors of papers that publish in this area. Also advertise and launch survey campaign Sept-November 2025 and leverage inperson ISPAD meeting for promotional purposes
e. Project Plan/ organization
i. Created sections that focus on tasks. Each section led by 1-2 members
ii. Engage 2-3 fellows from across regions to help spearhead and lead project efforts
iii. Inquire admin/ funding support from ISPAD
iv. Agreed on first of 3 sections
